Information clause regarding the processing of personal data in the Commune office in Galewice
In connection with the entry into force on May 25, 2018 of the provisions of the Regulation of the European Parliament and of the Council
of the European Union 2016/679 of April 27, 2016 on the protection of individuals with regard to the processing of personal data and on
the free movement of such data, and repealing Directive 95/46 / EC (General Data Protection Regulation), we would like to inform you that
pursuant to Art. 13 RODO, from May 25, 2018, you will have the following rights related to the processing of your personal data by the
Galewice Commune Office.
1.The administrator of personal data is: the Head of the Galewice Commune with headquarters in Galewice at ul. Wieluńska 5

2.Under Art. 37 sec. 1 lit. a) GDPR.The Administrator (ADO) has appointed the Data Protection Officer (DPO), who on his behalf
supervises the sphere of personal data processing jointly for the Galewice Commune Office and subordinate units listed below:
Communal Social Welfare Center in Galewice;
Communal Cultural Center in Galewice;
Communal Kindergarten in Galewice;
Primary School in Galewice;
Primary School in Niwiska;
Primary School in Ostrówek;
Primary School in Biadaszki;
Primary School in Osiek;
Primary School in Węglewice;
Junior high school in Galewice.
The Data Protection Officer is Mr. Michał Bartela, phone no. 501 011 826,email: inspektor@myiod.pl 3. Purpose of processing personal dataThe scope of activities of the commune self-government includes the performance of communal public tasks not reserved by
statute for the benefit of government administration bodies. The Galewice Commune Office collects your data in order to perform the
tasks resulting from the provisions of law and the Statute of the Galewice Commune, in particular from the Act of March 8, 1990 on
the commune self-government (Journal of Laws of 2018, item 994). The legal basis for the processing of your data results from a number
of competence (substantive) acts as well as obligations and tasks commissioned by institutions superior to the Commune.
4. Data storage periodPersonal data will be stored for a period not longer than it is necessary to achieve the purposes of personal processing, moreover,
this period may be longer due to processing as part of a specific procedure provided for by law, as well as due to processing for
statistical purposes and for archival purposes on the terms set out in in accordance with the provisions on the national archival
resource and archives.5. Right to restriction of processingIn connection with the processing of data for the purposes indicated in point 3, your personal data may be made available to other
recipients or categories of recipients of personal data.The administrator processes your personal data in a strictly defined, minimum scope necessary to achieve the purpose referred to above.
In particular, the Administrator may transfer / entrust your data to other entities. The basis for the transfer / entrusting of data are
legal provisions (e.g. the judiciary, tax administration, institutions related to the service of broadly understood EU funds, entities
related to the service of the social sphere - ZUS, PFRON) or properly constructed, ensuring the security of personal data,
data entrustment agreements to processing (e.g. with entities from the ICT and telecommunications sector, data processing).
6. Data Subject RightsIn connection with the processing of your personal data by the administrator, you have the right to:access to your personal data,
demand their rectification,
deletion or restriction of processing,
object to processing.

at any time without affecting the lawfulness of the processing which was carried out on the basis of consent before its withdrawal.7. The right to lodge a complaint with the supervisory authorityYou have the right to lodge a complaint with the supervisory body dealing with the protection of personal data in the
Member State of your habitual residence, place of stay, place of work or place of the alleged infringement. President of the Personal Data Protection Office (PUODO).
